What the inspector wrote

On 1/21/26 at 1:30pm, Licensing Program Analyst (LPA), Stephanie Li conducted an unannounced Case Management inspection for follow up on capacity increase, room addition and components served change application details. Upon arrival, LPA met with Director Evette Estrada. Application was forwarded to LPA on 10/30/25 with previous directors name listed. A complaint was received that placed the application on hold. The complaint has been closed and application is under review again. Facility is requesting to be licensed for 18 toddlers (18-36mths), 68 preschool children (2-6yrs), and 30 school age (Tk-8 th grd) and requesting to add the Dome room to licensure for use of TK age children. Hours of operation will be Monday Friday 7:00am- 6:00pm . Present during today’s inspection were 32 children with 6 staff. Appropriate ratios and supervision were observed. All indoor and outdoor space utilized for the children was inspected today. LPA followed up about fire inspection. Per Director, fire inspectors came out and said they were cleared but the fire inspector required proof of occupancy permit before giving formal clearance. Per director, inspector emailed them in November 2025 to request licensing to submit a new STD850 once they have submitted the occupancy permit proof. Email was forwarded to LPA. LPA advised Director to follow up with submitting the occupancy permit fire inspector and cc LPA S Li so that updated STD850 can be sent. During today’s visit, LPA observed outdoor TK yard space, TK bathroom, and Dome Tk room and application and supporting documents originally submitted with application. LIC200A list previous Director name and request school k-6 th . Per Director, they would like to request school age TK-8 th . LPA printed new LIC200A and had director update it with change of capacity and component change, new directors name, school age tk-8 th , and add signature and date. Completed updated LIC200A was received during visit. page 1 of 2 LPA advised that rusty metal bins and other hazardous materials (sitting water buckets, broken splintered fences, long metal pole, spider webbed equipment) in the tk yard needs to be removed prior to inspection. LPA advised that walkway to the bathroom from driveway will need an additional gate to prevent children from having access to driveway during bathroom transitions. LPA advised that staff are required to maintain direct supervision of the children at all times during bathroom transitions, indoor and outdoor activities. LPA informed Director that once occupancy permit is submitted to fire inspector, licensing will forward new STD850 to Fire Inspector. Once approved fire clearance is received, CAB LPA C Green will be conducting licensing inspection to approve licensure of new component. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the Director Evette Estrada.
