What the inspector wrote

Licensing Program Analyst Warren Birks conducted an announced inspection at the facility noted above and met with Directors Reyna Villalobos, Director Adriana Ortiz and Senior V.P. of Youth Development Christopher Jefferson. The purpose of the inspection is to inspect a new classroom change (from Bungalow #1 to classroom B42) and inspect the already licensed cafeteria/auditorium classroom. Applicant is seeking to provide care for 96 school-age children. The program is located at Maude Price Elementary School. The program provides care for school-age children Monday through Friday from 6:30am to 8:30am and after-school from 2:00pm to 6:00 pm. Early out date is from on Wednesdays at 12:00pm to 6:00pm. The program will be operating in classroom B42 and in the school's cafeteria/auditorium. All areas identified on the facility sketch were inspected. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. The program has outdoors hooks for children's belongings outside the classroom. The facility will provide children with snacks, which will be supplied by the elementary school cafeteria. First Aid supplies are located underneath the desk and in the kitchen near auditorium. Per Director, medication is only administered to a child when accompanied with a doctor's note and medication is stored in a locked medication box located in a locked file cabinet. Disinfectants, cleaning solutions, and other items that are dangerous to children, were inaccessible to children in all areas. Director stated that there are no poisons stored at the facility. All storage containers for solid waste have tight-fitting covers that are kept on and in good repair. Per Director, the isolation area is located in the northeast part of the classroom where a mat is available for resting. The staff restroom is readily available if an ill child requires it. Age appropriate sinks, toilets, and urinals were inspected for availability and good repair. General sanitation was observed. Availability of indoor drinking water was also observed (Water dispenser). Smoke detector is hard wired to the building and the carbon monoxide detector was tested and is operable. The facility was granted a fire clearance for the requested capacity of 96 on November 03, 2023. The fire extinguishers are fully charged and were last serviced on July, 2025. Outdoor playground equipment is in a safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. YMCA does not permit children to use climbing equipment (All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that could absorb a fall). There is adequate shade in the outdoor play yard. The facility fills up water containers and cups from the water dispenser to use for children drink freely. There are no bodies of water were observed on the premises. Facility representatives were reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Items discussed during this inspection: 1.Licensing does not prohibit the following: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238(g)(2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both. 2.Food Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request. 3. Current Children\342\200\231s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request. 4.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present. designated Staff: The name of the child care center Director or fully qualified teacher(s) designated to act in the Director's absence must be on file. 6.Qualifications: Educational background, training, and/or experience for each staff present must be available for review. 7.Children\342\200\231s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. 8.Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. 9.Records to be Maintained at the Facility: Child Care Centers (LIC 311A). 10. AB1207: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees are to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department\342\200\231s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/ . LPA Birks advised facility representatives that the children need to be within the direct care and supervision, including visual supervision of staff at all times especially the cafeteria/auditorium. LPA Birks also advised the facility representatives to access forms, regulations, quarterly updates, and Provider Information Notices (PINs) on the Department website at: www.ccld.ca.gov . Based on measurements and fire inspection, the maximum capacity for the proposed program would be 96 children and classroom B42 is currently approved for 36 based on common Elementary classrooms of similar size on the premises. NOTE: Per Title 22: The capacity per room for a school-age child care program that meets the requirements of Health and Safety Code Section 1596.806 shall not exceed the number of children for which a room of this size is commonly approved for school use during the school day. LPA informed the facility is required to adhere to the terms and limitations stated on the license. No deficiencies were cited during today's inspection. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with Director Reyna Villalobos, Director Adriana Ortiz and Senior VP of Youth Development Christopher Jefferson.
