What the inspector wrote

An unannounced Required Annual site inspection was conducted by Licensing Program Analyst (LPA) Bardo Baluyot who met with Yiyun Chen. LPA conducted a COVID risk self-assessment upon entry. This facility is a Head Start program that currently operates from 8am-12pm & 12:30-6:30pm (2nd bungalow McKinley-1), 8-11:30am & 12:15-5:15pm (first bungalow-McKinley-2) and 8:45am-12:15pm (Surround Care bungalow-McKinley-3). Upon arrival, LPA observed a total of 2 staff with 11 children outdoors from McKinley #3 and a total 33 children with 8 staff present. LPA found that facility is within ratio. Care and supervision was evaluated to determine if the basic needs of children are met. LPA observed during this inspection that facility is in compliance of this. There are three bungalows on this property. The first bungalow (Head Start), the second bungalow behind it (Head Start) belong to this licensed facility. There is a third bungalow that operates an Options- Surround Care program with a separate license 198003209 and is used as McKinley-3. Furniture and equipment were inspected for age appropriateness and its condition. Telephone service, heating, lighting and ventilation were evaluated. Storage space/area for children's belongings were inspected. The condition and age appropriateness of children\342\200\231s sinks and toilets were inspected. LPA evaluated general sanitation and cleanliness of facility. Availability of indoor drinking water was observed in form of water water jug and disposable cups, as well as children's personal water bottles. Carbon monoxide/smoke detectors were tested in all bungalows and found to be operational. Outdoor area and equipment was inspected for safety, cushioning material, condition and age appropriateness. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water. Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness. A review of cleaning and food supply storage areas was made. LPA inspected First Aid kit\342\200\231s supplies for completeness. All required postings were observed in each classroom including each staff members' current CPR/1st Aid certificates. Disaster drills were conducted in September 2022. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Children's files and sign in and out sheets were reviewed. No deficiencies cited during today's visit No children or staff files were reviewed during today's visit. Files are kept in Options State Preschool's main office located at 1330 Potrero Ave, South El Monte, 91733. (626) 459-4299 Exit interview conducted and report was reviewed with Teacher, Manna Ho. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
