At 12:30PM. The Licensing Program Analyst (LPA), T. Tran, conduct an unannounced site visit at MAOF-Child Care Center Maywood to follow up on a Case Management Incident Continuation occurred on 3/09/2026 pertained to a parent's concern about an allegation that a child was slapped by staff. Upon arrival, LPA met with Site Supervisor, Marcela Torres. Based on records reviewed and interviews conducted. There was no evidence or corroboration supporting the allegation information, The report concluded the incident did not appear to violate Title 22 Personal Rights regulations. No further investigation is required for the incident reported. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Marcela Torres.
