What the inspector wrote

On 12/09/2025, Licensing Program Analyst (LPA) Stephanie Li conducted a collateral visit for Duarte Unified School Dist-Maxwell site at Duarte Child Development Center located at the Andres Duarte elementary school campus. The school campus has been closed and only the preschool center and Child development center are present. The purpose of this visit is to review staff and children files as they are stored at the child development office site. LPA met with Director Sofia Valdez, who provided LPA with the staff and children\342\200\231s files who were present during the annual inspection on 12/09/25. Children records were reviewed for completeness and logged on LIC857. One child was observed to have notes regarding food allergies to almonds, cashews, and walnuts. No epi-pen was observed during inspection. Per interviews with teachers, no epi-pen was provided for the child. Special meals accommodations, plan to accommodate is to avoid all nuts and liquid milk for child\342\200\231s meals from the nutrition department. LPA advised to obtain a written statement from parents stating that epi-pen is not required due to minor reactions. All staff have received an active criminal record clearance as a condition of their employment with the Duarte Unified School District. Staff records were reviewed for completeness. Inspection of required forms were made. LPA logged Review of Staff Records (LIC 859) during this inspection. All staff have completed Mandated reporter training and Cpr first aid certified. During todays visit, Director informed LPA that the Andres Duarte school campus has been sold and the preschool on this site will need to relocate to another site by next school year. LPA consulted about steps for relocation and advised to submit application for relocation by end of this school year so there isn't interruption in services for children. page 1 of 2 To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. At this time, the facility is in compliance with California Code of Regulations Title 22. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Sofia Valdez.
