What the inspector wrote

At 9:15 AM Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced annual random inspection. LPA met with Teacher Leticia Gutierrrez who led analyst into the facility. Program facilitator Claudia Casillas arrived at the facility at 9:30 am and took over the inspection. This is a preschool age head start program licensed for 20 children which operates an AM session Monday \342\200\223 Friday 8:00 am to 11:30 am with 14 children enrolled and a PM session Monday- Friday 12:00 pm to 3:30 pm with 6 children enrolled. All areas identified on the Facility Sketch were inspected. Facility is located in 2 classrooms MV # 1 and MV# 2 on the Monte Vista Elementary School Campus. Classroom MV# 2 is used for the Early Head Start Home Based program twice a month for socialization and parents are present during that time. There are 2 waivers on file- 1 to share bathrooms and 1 for Early Head Start to walk through preschool yard. The following staff was present during this inspection: MV #1 Staff # 1 and Staff # 2 with 11 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license. LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. Classroom was inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, isolation area is in the MV# 2 child stays with Family Service Worker. --- pg 1 of 5 ----- Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted. The facility provides breakfast, lunch for the AM session and lunch and snack for the PM session. Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Per Program Facilitator meals are dropped of daily by the cafeteria in the school. Meals are stored in the refrigerator located in the kitchen dividing both rooms. All food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available in the classroom via water jug and dixie cups. Personal water bottles are taken outside for children to drink. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. LPA observed that there is smoke and carbon detectors in the facility and were found to be operable. The last Disaster and Fire Drill was conducted on 11/13/2024. Per program facilitator since the facility operates two sessions children do not take naps. LPA observed individual storage for children. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. ----------------------------------------------- pg 2 of 5 -------------------------------------------------------- Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. All staff have received an active criminal record clearance as a condition of their employment with the Mountain View School District LPA did not review complete staff files during the inspection due to the staff files being stored at the administrative office located at Mountain View School District. LPA will review staff files at a later time and date. Children's emergency records were reviewed; children's health records are kept in the administrative office and will be reviewed at a later time and date. If files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files. AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. LPA discussed the safe sleep regulations with licensee facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. --------------------------------------------------- pg. 3 of 5 ------------------------------------------------------------ For IMS information see PIN 22-02-CCP. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
