What the inspector wrote

At 9:00 am Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced case management- incident inspection to the above facility. The purpose of this inspection was to follow-up on an incident that was self- reported to the department on 8/25/25.. LPA met with Teacher Elisabet Macuil who the purpose of the inspection was announced.Program facilitator Tommy Velazquez arrived at 9:52 am. Census was taken. On 8/25/25 an incident was self reported to the department. Per Incident reported, on 8/22/25 during free time inside the classroom Child # 1 was hopping on carpet area- and sat down. Child # 1 then began to cry and limped towards teacher, pointing to left foot. Incident report was reported to the department within 24 hours. LPA conducted interviews regarding incident with staff. Staff corroborated that child was playing in large carpet area, child was hopping and sat down. As child sat down they complaint about their foot. Ice was applied and parents were called. Child was taken to the doctor by parents and came back a few days after with no restrictions. Per Staff they remind children to use walking feet in the classroom. At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the facility representative Tommy Velazquez. ----------- pg. 1 of 1 -----------
