On 05/02/2024 at 3:15 pm Licensing program Analyst (LPA) Shushanik Safaryan conducted an unannounced Plan of Correction visit to the above facility. At 3:00 pm LPA met with Facility Representative Diane Connell who guided the LPA on a tour of the facility . This licensed facility is also located on a shared campus with Almansor Academy. Preschool program operates Monday through Friday from 07:00am to 05:30pm. Program consists of 2 preschool classrooms and 1 toddler classroom. Per Facility Representative there are 51 children enrolled. The purpose of this inspection is to ensure that the facility follows Title 22 Regulations and the deficiencies cited on 03/28/2024 were corrected. During this visit 27 children were present with 4 staff members . During the visit on 03/28/2024 LPA observed preschool classroom with an emergency exit accessible to the children in care and cracks , holes on the surface of the preschool play area .Upon arrival LPA toured the facility and took pictures of corrected areas . LPA observed door alarms placed on the doors which was tested and was operable during the inspection . Next LPA toured the play area and observed holes and cracks fixed .(Pictures were taken ). Letters of Deficiencies Citations Cleared were provided for deficiencies corrected. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted, and a copy of this report was provided to Diane Connell along with Appeal Rights.
