What the inspector wrote

On November 10, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection at the above facility. LPA met with director, Angela Barlow who guided LPA on a tour of the facility. LPA observed 30 children with 9 staff members. The purpose of the inspection is to follow up on an incident report was occurred on 10/31/2025 and was reported to the department on 11/03/2025. The incident was reported timely. During the inspection LPA interviewed Staff #1 (S1) to Staff #2 (S2). LPA attempted to interview Child #1 (C1), but C1 did not want to be interviewed. LPA observed the area where the incident occurred and obtained relevant documentation. Based on interviews, S1 stated that on 10/31/2025 the children were playing on the play structure when C1 jumped and landed wrong on their foot. Per S1, they assisted C1 to get up and noticed C1 was favoring their right foot. Per S1, they called Parent #1 (P1) and notified P1 through Brightwheel App as well. Per S1 and S2 the facility was in ratio at the time of the incident and provided documentation to LPA. Per S1 and S2, P1 arrived at the facility and assisted C1 in the Halloween parade and took C1 to seek medical attention after the facility's event. Per S2, the facility was notified on 11/01/2025 that C1 sustained a fracture on their right foot. C1 returned to the facility on 11/03/2025 and a doctors note was provided to the facility. There are no deficiencies being cited as the incident appears to be an accident that occurred quickly and was unable to be prevented. Per S1, their classroom has implemented discussion about safety when using the play structure. An exit interview was conducted and a copy of this report was provided along with Notice of Site Visit. Notice of Site Visit must be posted for 30 days.
