On April 8, 2025, at 10:15 AM, Licensing Program Analysts (LPAs) A. Carter and J. Duran conducted an unannounced Annual Random inspection and met with Facility representative (FR), Carolyn Lapitan, Director. LPAs disclosed the purpose of the inspection and was granted entry into the facility. FR was provided a copy of the Facility Entrance Checklist. LPAs were taken on a guided tour of both the indoors and outdoors of the facility. Teacher-child ratios were observed, and staff names recorded. All center staff that were present during today\342\200\231s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. The electronic sign-in and out sheets were reviewed. The hours of operation are 7:30 AM \342\200\223 5:00 PM, Monday - Friday. Facility is currently licensed for a capacity of 125. At the initial start of the site visit, there is a total census of 80 children and a total census of 13 teachers present. The facility is made up of 4 classrooms. Room 101: had 18 children and 2 staff, Room 102: had 18 children and 4 staff, Room 103: had 21 children and 4 staff, Room 104: had 23 children and 3 staff present at time of inspection. LPAs observed all posting requirements for operation on the posting board and the following documents were observed: LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent\342\200\231s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule. At 10:25 AM furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children napping equipment met the requirement regulation at time of inspection. Napping equipment and bedding were inspected and found to be clean and in operable condition. Per Director, bedding is washed weekly at the facility. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. REPORT CONTINUES PAGE 2 OF 4 LPAs observed 2 bathrooms available in each classroom that has 2 toilets and 2 sinks. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPAs observed cubbies with wooden toys, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPAs observed carpets and age-appropriate tables and chairs in each classroom. LPAs observed first aid kept in each bathroom of each classroom. First Aid supplies were inventoried and were found to be in compliance with regulation. Isolation area for sick children is the Isolation room next to directors\342\200\231 office with attached bathroom. The facility administers medication, LPAs observed medication to be locked in a cabinet in the office inaccessible to children in care. Medication is administered on a as needed basis. Food menus were observed to be posted and reviewed. Facility provides breakfast, lunch, and PM snack. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. At 10:35 AM the food preparation area was toured and found in compliance for safety, cleanliness and proper storage. Each child brings their own water bottle which is labeled with their names. Drinking water is also provided via water fountains in each classroom and outside. LPAs confirmed Carbon monoxide and smoke detectors were present and determined to be in operable condition. Fire extinguishers were observed as serviced on 01/03/25. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. A review of medication policy, including administering, labeling, storage, and records was made. Disaster drill log observed with last drill conducted on 10/17/24 per observation. At 10:45 AM, outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. LPAs observed bikes, sandbox area, sensory tables, a play structure with a plastic slide, and other age-appropriate toys around the play yard. Required shade and fencing were inspected. LPAs advised that the children need to be within the direct care and visual supervision by the teachers at all times. The children have access to drinking water via water coolers observed by LPAs. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection. At 11:15 AM, staff and children\342\200\231s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. LPA observed current CPR and First Aid Certification for center staff. CPR card expires on 07/06/26 for Staff Member #1. Role and responsibilities of being a Mandated Reporter were reviewed. FR was advised how to access forms and Regulations online at www.ccld.ca.gov . to ensure the most current information regarding regulations for all those who assists in providing care. REPORT CONTINUES PAGE 3 OF 4 The following were discussed: LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 04/08/25 . A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Carolyn Lapitan. Appeal Rights Given
