What the inspector wrote

Licensing Program Analysts (LPAs) A. Carter and J. Duran conducted an unannounced case management inspection to follow up on an incident that was reported to the Department. Upon arrival, LPA met with facility representative (FR) Carolyn Lapitan who provided LPAs on a tour of the inside and outside of facility. Census was taken. The facility self-reported an unusual incident to the department on 07/14/25 regarding an incident that occurred the same day where a child was left unsupervised inside a classroom. A staff (S4) observed a child (C1) in the quite space on their side of the shared classroom. During today's visit LPAs interviewed the child and staff involved. Based on the information gathered during the interviews there were 14 children with three staff in the classroom. Around 9:20am, during the transition from inside to outside, C1 was left in the quite space for a few minutes while the rest of the class was outside. A teacher inside the conjoined classroom observed C1 alone and returned them to their group. According to the available information, this incident was the result of a Title 22 violation for lack of care and supervision. Facility conducted a staff meeting/training to discuss proper care and supervision during transition time. The copy of the agenda of the meeting/training regarding care and supervision and the list of attendees with their signatures were provided to LPAs on today\342\200\231s visit. On 07/24/25, LPAs received all the required POC documents, and all citations have been cleared. Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health & safety. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted and report was reviewed with facility representative, Carolyn Lapitan.
