What the inspector wrote

On 5/4/2026 Licensing Program Analyst (LPA) A. Nunez conducted an unannounced Case Management inspection. Upon arrival LPA met with Director, Rebecca Smith. A tour of the facility was conducted, and census was taken and the total census at that time observed was 19 children and 6 staff members. The facility was operating with staff-child ratios and within licensed capacity. A review of the Facility Personnel Report Summary conducted on today’s date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the visit to the facility on 5/4/2026, LPA discovered that the facility failed to report an incident that happened on April 24, 2026. Based on 3 staff interviews, it was stated that on April 24, 2026 Child#1 (C1) slip and fell on his bottom and fell back on the classroom carpet. The fire department and ambulance came to the facility to inspect C1. The facility did not report the incident to the Orange County Child Care Licensing Division Regional Office. Based on staff interviews and record reviews, the facility was not in compliance with California Code of Regulations, Title 22, Division 12 & Chapter 1, 101212(d)(1)(c) Reporting Requirements is being cited; see LIC 809-D. LPA provided the facility with the LIC 624 form and with the Title 22 regulation section 101212 for reference. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Rebecca Smith. Appeal Rights discussed. Director was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. End of Report

Facility failed to report to licensing office about the incident that took place at the facility on April 24, 2026 when the fire department came to the facility to inspect C1. This posed a potential risk to the health and safety of the child in care.
