What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 4/28/2026 and was reported to the Department on 5/1/2026. LPA met with Director Angeline Arias. Reason for the visit was explained. There were 45 children and 21 staff. On 5/1/2026, an unusual incident report was made to the Department regarding an incident where a child was injured during dismissal time, child was sitting at table waiting to be called during pick up and child jumped/bucked upwards falling backwards, hitting the back of his head on the wall cubbies. Child sustained a an inch long gash at the back of his head, middle area. First aid was immediately applied and 911 was contacted due to laceration and bleeding. Paramedics offered to transport child but parents declined and took child to nearest ER. During todays visit, LPA interviewed director. Per Director and classroom teacher, reviewed ouch reports and district head injury log, and reviewed child’s file. Per director, no medical notes were provided by parents. Per parents, child needed 5 staples. Child was absent for 2 weeks after injury. Pre-planned absences and trips were planned prior to injury. Parents reported child went to Disneyland the next day and went on other planned trips. Child returned 2 weeks after incident without restrictions and staples were removed prior to his return. LPA observed classroom furniture set up and observed that table about 4 feet away from cubbies. Per teacher, table was closer when the injury occurred. LPA observed 12 children in the classroom and there is enough spacing between classroom furniture. No follow up is necessary at this time. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Angeline Arias.
