What the inspector wrote

On 06/10/2025, Licensing Program Analyst (LPA) Kruz Long conducted an unannounced Case Management visit to the facility for the purpose of issuing a citation. LPA met with Victoria Casas, Assistant Director and explained the purpose of the visit. A Covid 19 risk assessment was conducted. There are currently 40 children enrolled. 11 Staff and 22 Children are present in 4 different classrooms during today's visit. The department investigated a complaint received on 03/14/25. The complaint allegation pertains to personal rights violations of children in care and an internal investigation was conducted by the facility. Although the outcome of their investigation involved the suspension of staff, the facility did not feel the incident called for further action, including reporting the incident to the department. A complaint investigation conducted by the department resulted in a substantiated finding. Based on the information gathered, LPA found that the facility failed to report the incident to the department as required. The facility is being cited a Type B deficiency in accordance with California Code of Regulations, Title 22, Division 12, Chapter 1, Article 06, Section 101212 (d)(1)(D). Definitions are being cited on the attached LIC809D page. An exit interview was conducted and a copy of this report and appeal rights was provided to the Assistant Director. A Notice of Site visit was also provided and must be posted for 30 days.

Two of eight staff interviewed also indicate that on more than one occasion they have witnessed S8 mistreat infants. S7 admitted to mistreatment of children.
