What the inspector wrote

On 08/20/2025, Licensing Program Analyst (LPA) Kruz Long conducted an unannounced case management visit. LPA met with Marcela Torres, Director and explained the purpose of the visit. There are currently 40 Children enrolled. 27 Children and 12 Staff are present in 4 different classrooms during today's visit. The reason for today\342\200\231s visit is to investigate an incident that occurred on 08/18/2025 and was reported to the department on 08/19/2025. The self-reported incident is regarding food services. During the course of the investigation, LPA Kruz Long interviewed Staff #1 (S1) in the office, toured the classrooms, obtained and reviewed a copy of Staff statements and Child #1's (C1) records. According to the interview with Staff #1(S1), Staff #2(S2) gave C1 low fat milk during breakfast time not aware that C1 is lactose intolerant. Written statements date 08/18/25 by Staff #3 and #4 who were both present at time of this incident verified that this incident occurred. LPA review C1's medical record and parental request which indicate C1 is to be provided lactose free milk. Based on interview conducted and record review, the deficiency was cited in accordance with the Title 22 of the California Code of Regulations and Health & Safety Codes. See LIC809D page for documentation of deficiency. An exit Interview was conducted and a copy of this report and appeal rights along with Notice of Site visit was provided to the Director.
