What Happened
Incident from 01/20/2026 where two children were out of visual observation near fence/gate during outdoor play while staff S1 was at restrooms. Children attempting to retrieve a ball. Duration unclear but children were out of sight for unknown period. This is the second supervision-related incident since December 2025. Type B citation issued for lack of supervision. Added staff coverage recommended.
