What the inspector wrote

On 08/05/2025 at 10:15 am, Licensing Program Analyst (LPA), Crystal Green conducted an announced Case Management Licensee Initiated inspection to increase capacity. LPA met with Licensee Representative, Monica Jarvis and Cynthia Guerra. This facility is currently licensed as a HeadStart program with a capacity of 24 children serving ages 3 until entry into kindergarten in the HeadStart Classroom. The Licensee, Hacienda La Puente Unified School District, has requested to increase their capacity to 44 children from ages 2 to 5 years old by adding one additional classroom called (K3). The hours of operation will be Monday to Friday (Half Day Program) 8:00 am 11:30 am and (Full Day Program) 8:00 to 2:30 am. This facility is located on the west side of the main office of Lassalette Elementary School. To gain access to the school there is a side gate for parents to drop-off and pick up children. The gate will be kept locked during operating hours and are only unlocked when children are being dropped off and picked up. The office will be located in the HeadStart classroom, which will also serve as the ill isolation area for children. Children sign in and out will be conducted at the front entrance the classroom. There were no children present during today\342\200\231s inspection. Per Licensee Representative, the program will begin on 08/06/2025. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed the Licensee Representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. Report Continues Page 1 of 3. LPA continued to tour the facility and inspected the existing classrooms. Total indoor activity space obtained during this inspection measured 2,176.58 square footage, which is sufficient to accommodate their requested increase in capacity. LPA observed all indoor activity spaces to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water will be available in the classrooms via filter water jugs and child\342\200\231s water bottles. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 8/04/2025. LPA observed a total of 5 sinks and 4 toilets available for children\342\200\231s use. These are sufficient to accommodate the requested capacity of 44 children. There is a kitchen area located with the HeadStart classroom that is off-limits to the children. There is a staff restroom located within the HeadStart classroom. The facility currently has a fully fenced playground area located adjacent to the HeadStart classroom. Fencing is made of chain-link which was observed to be at least four feet high. Per Licensee Representative, no changes have been made to the outdoor space. LPA did not obtain measurements for the outdoor space. There is also an approved outdoor waiver allowing for the sharing of the outdoor space with the elementary school. Shade is provided via a fixed shade structure. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via child\342\200\231s personal water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Facility Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Licensee representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. No corrections are needed. Upon a final file review, a capacity increase requesting to serve 44 children from ages 2 to 5 years old in (2) classrooms: K3 and HeadStart will be granted. Exit interview conducted and report was reviewed with the Licensee Representatives, Monica Jarvis and Cynthia Guerra. A notice of site visit was given to Licensee Representative, Monica Jarvis and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
