What the inspector wrote

On this date Licensing Program Analyst (LPA) Kamile Martin went to Barfield Child Development Center for the purpose of observe the child whom received an injury reported to the office on 10/16/25. On 10/13/25 Ch received an injury to the forehead, Ch fell face forward while attempting to kick a ball. The injury resulted in a bump and scratch on the forehead LPA spoke with teacher Mary Blankson who described the nature of the incident and stated the impact was hard when child fell forward. An Ice pack was applied to the area and parent was notified. Parent picked child up from care that day and received and ouch report of the injury . He was out for a few days before returning to school. LPA asked teacher Mary was child cleared by a medical professional teacher states she is unsure. LPA called school Nurse Alma Gaitan to confirm childs clearance to retune to school. Alma did not answer but a message was left. Teacher Mary called attendance to confirm ch was cleared to return to school but recieved no answer as well. Per teacher child is functioning well. LPA will reach out to School Nurse to confirm School Clearance. Based on the information obtained during this investigation, no follow up is necessary regarding the incident reported. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00 Exit interview conducted and report was reviewed with Mary Blankson.
