What the inspector wrote

Licensing program analyst (LPA) Kamile Martin conducted an unannounced Case Management inspection due to an incident that occurred on 10/24/25 and was reported to the department on 10/28/25 via email. LPA met with Teacher Rebecca Ramirez Sapien, Teacher Rocio Zuniga and Teacher Arcelia Guerrero. LPA spoke with Teachers, whom the reason for the visit was explained. LPA went on a self -guided tour of the facility. There were 3 children present with parents and 3 staff present. This is a Early Head Start Home based program which serves pregnant mothers, Infants and toddlers up to age 3. The facility was observed to be operating within the license capacity limitations. On 10/28/25 an unusual incident report was made to the Department regarding a childs injury during a group connection, child walked into the handrail and bumped her forehead. Per parent child may have not seen the rail due to the sun in her face. Childs father was present and observed the incident. An ice pack was given to the parent to apply to the child\342\200\231s injury. Parents stated they will take child to have a follow up exam. Supervisor Eileen Carrillo Lau was on site during child\342\200\231s injury, Teacher Rebecca Ramirez Sapien wrote the (UIR) Unusual Incident Report. LPA Kamile Martin observed area where child was injured. Upon observing the area where child was injured Teacher Rebecca states the supervisor is looking into a solution to provide cushion for the metal bar child was injured on. Child has not returned to care although parents took child to the doctor for follow up a medical clearance has not yet been received. LPA called Nurse Alma Gaitan and left a voice mail message , and Leticia Rodriguez Office Manager and no was message left. At this time there are no deficiencies being cited . A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00 Exit interview conducted and report was reviewed with Rebecca Ramirez Sapien Teacher.
