• Sherry Keller not yet disassociated as required by 04/29/2024 exclusion letter - Director Head instructed to disassociate immediately on 05/20/24, submitted disassociation letter by 05/21/24
• Sign in/out correction not properly documented - reminder document provided to parents but no date, director signature, or list of parents who received
• Children commingling issue unresolved - infant had been enrolled and in care with preschool children despite separate program requirement
• Only 18 of 43 enrolled children have LIC 9224 acknowledgment form for Type A citation dated 05/08/2024
• Director prefers in-person delivery of license reports and acknowledgment forms to parents
• Director had office meeting with department on 04/09/2024 but has not made agreed corrections as of 05/20/24
