Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management Annual Continuation inspection at the above facility on 11/21/25 at 10:19 a.m. LPA met with the facility representative who guided analyst on a tour of the facility. There were 21 children with 04 staff. Children and staff files were not available for review during the annual/random inspection conducted on 09/09/25 as they are stored at the Rosemead School District Office. On 11/04, LPA reviewed (5) random children\342\200\231s files and observed 2 out of 5 children were missing LIC 701. On 11/21, LPA reviewed (5) staff files and observed 1 out of 5 staff files missing LIC 9052. LPA provided technical violations. Children and staff names were documented on the LIC 857 and LIC 859, and a copy was provided to the facility representative during the inspection. At this time, the facility is in compliance with California Title 22 Regulations. Therefore, there are no deficiencies being issued today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Shaunna Homann.
