What the inspector wrote

Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management Incident inspection at the above facility on 11/21/25 at 11:25 a.m. LPA met with the facility representative who guided analyst on a tour of the facility. There were 21 children with 04 staff. Upon LPAs arrival the facility representative informed LPA that there was an outbreak of Hand, Foot, and Mouth. According to the facility representative, there were 11 confirmed cases and Centers for Disease Control and Prevention (CDC) was contacted by the elementary nurse. Per facility representative, the district moved staff and children to another classroom temporarily to deep clean and disinfect the entire classroom. Facility representative also stated that children are not sharing toys, daily deep cleaning is taking place, wiping chairs, tables, mats, fogging the classroom after hours. CDC provided resources and recommendations that were followed. LPA was provided with a copy of the children that were confirmed with HFM which included date of onset, date returned, and comments. LPA observed that the first case was on 10/28 -11/05. Per facility representative, parents were notified and given a HFM Notice which a copy was obtained during the inspection. LPA observed that the classroom currently has no rugs since those were thrown away. The facility has purchased new rugs and are expected to be delivered soon. The facility hasn't had any other confirmed cases for over 10 days. The incident was not reported to the Department within the required 24 hours of occurrence. LPA provided a technical violation and a copy of the reporting requirement regulation. At this time, the facility is in compliance with California Title 22 Regulations. Therefore, there are no deficiencies being issued today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Shaunna Homann.
