What the inspector wrote

Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management Incident inspection at the above facility on 04/23/26 at 11:15 a.m. The purpose of this inspection is to follow up on an incident that was reported to the Department with 24 hours on 04/17/26. LPA met with the facility representative Queta Morales who guided LPA on a tour of the facility and census was taken. During this inspection, LPA interviewed Staff 1 (S1). On 4/16/26, during indoor play time, child 1 (C1) was in the discovery-science area and tripped on his “feet”. C1 fell and hit his right eyelid on the light table. S1 assessed the child, observed minimal blood, and provided first aid. The nurse went to the classroom and observed a small laceration on the child’s right eyelid. Parent of C1 was notified and the child was taken to see a medical professional and was discharged on 4/16/26 with no restrictions. Child returned to the facility on 4/17/2026. Interviews conducted with S1 revealed that they were in the science area next to C1. C1 was playing in the light table stacking and lining up letters. Per S1, C1 was blowing on the letters so that they could tip over when the child was observed to take a step back and then a step forward. When the child took a step forward the child tripped with his feet and scratched his eyelid. S1 recalled that there was no thump heard when he observed C1 fall forward the light table. C1 began to cry and went towards S1. S1 assessed C1 and observed minimal blood. Per S1, they cleaned the area and provided an ice pack. Soon after the child received an ice pack there was no more blood. S1 called the program’s nurse and also notified the parent of C1. Page 1 of 2 The program’s nurse arrived at the classroom to assess the child. C1 continued to engage in the classroom activity until their parent arrived at the facility. The parent of C1 was asked to assess the child and was advised to take the child to urgent care or the Emergency Room (ER). C1 was taken to the doctor and returned the next day with a doctor’s note. C1 did not receive any stitches and had no restrictions. According to S1, C1 returned with no bruising or swelling as the injury was more of a scratch. Reminders for walking feet and rearrangement of the area where the table is located was done as precautionary measure. LPA toured the area where the incident occurred and observed age appropriate toys, materials, and equipment for children. Equipment is in good condition. LPA took a picture of the light table. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today. The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative, a civil penalty of $100 can be assessed. An exit interview was conducted, and a copy of this report was provided to the facility representative Queta Morales. Page 2 of 2
