What the inspector wrote

On August 25, 2023, Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced required 1-year inspection. A COVID-19 risk assessment was conducted prior to entering the facility. LPA entered facility through the Nelson elementary school\342\200\231s front office and walked towards the back of the school to enter through a gate into the preschool programs. LPA met with teacher, Melissa Garay and LPA took a census of 7 children and 2 staff in care. The two programs operating are a State preschool age program and Head Start preschool program. The Head Start currently did not have children in care as they are doing home visits. LPA called and spoke to the main office that is located off-site. At approximately 10:30 am Manager, Monica Jarvis and Site Supervisor, Ryan Martinez who oversee the preschool programs met LPA to facilitate the visit. Hours of operation: The days and hours of operation are for Nelson 01: Head Start: Monday-Friday, 8:00 A.M.-2:00 P.M. and State Preschool: Monday-Friday 8:15 AM - 11:15 AM and 12:00 P.M.- 3:30 P.M. Location: There are two portables, Nelson 1 and Nelson 2. The portables are located on the east side of Nelson Elementary. The portables and the playground are surrounded by a chain-link fence, separating it from the rest of the elementary school. Each portable is one large classroom with two restrooms for the children, one staff restroom, a kitchenette, and one office for the staff. The chain-link fence which surrounds the portable are kept locked during operating hours and are only unlocked when children are being dropped off and picked up. The children will enter through one door and exit through the emergency exit door in the back of the portable. To gain access to the school there is a side gate for parents to drop-off and pick-up children. page 1 of 3 All posting requirements were observed on the Parent Boards. In each portable, the Parent Board is located on the wall adjacent to the door through which the children will enter. In Nelson 01, a copy of the monthly menu is posted on the sign-in and out podium. All food is provided by the School District. In Nelson 01, breakfast will be served at 8:05 am, lunch at 11:00 am and P.M. snack at 1:30 pm; in Nelson 02, breakfast will be served at 8:15 am and lunch will be at 12:05pm and P.M. snack at 3:00 pm. Both portables have a kitchenette area which contains a full-size refrigerator. There is currently one child who requires medication which is currently stored in a locked cupboard up high in the kitchenette area out of reach of children. LPA observed medication not expired and required paperwork was stored with the medication. In both portables, medications which do not require refrigeration will be stored in a cabinet in the kitchenette area which is labeled "Medication" and locked by a chain and padlock which requires a key to open. Both portables have a First Aid Kit in the kitchen cabinet which is labeled with a sign. In both portables, the cleaning supplies are kept in the cabinet under the kitchen sink in the kitchenette; it is locked with a chain and a padlock. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP . There is a carbon monoxide and smoke detectors that are connected to the elementary school and are tested by the school district through-out the year. The last annual maintenance on the Fire Extinguisher was on August 8, 2023. In each portable, the children's restrooms have four toilets. The 2 child-size sinks are located outside of the restroom and of the kitchenette area. One of the sinks in each portable contains a drinking faucet, though teachers stated that they will use a drinking water filter with individual disposable cups so that the children don't have to use the faucet. The adult restroom will be used by staff. There is no napping equipment in the State Preschool but in the Head Start program, children nap and page 2 of 3 bring napping items from home and are sent back at the end of the week. Napping cots are sanitized each day and located in the classroom. LPA referred Site Supervsior and Manager to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. LPA provided Site Supervisor and Manager with PIN 21-21.1-CCP for written directives of Lead Testing. Site Supervisor and Manager were reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Site Supervisor and Manager were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. The deficiency listed on the following page were observed by the LPA and is being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. The deficiency that is being cited needs to be cleared to protect the children\342\200\231s health & safety. A notice of site visit was given and must remain posted for 30 days.
