What the inspector wrote

On 10/28/2025, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced case management inspection due to an incident which occurred on 10/15/2025. The written report was mailed and received to the Regional Office on 10/22/2025. LPA arrived at the facility and met with Director Caridad Greco, who guided LPA on a tour of the facility. Busy Bee RM: 16 children with a ratio of 2 teachers Sprouts RM:  16 children with a ratio of 3 teachers Sunflower RM: 19 children with a ratio of 3 teachers Rainbow RM : 11  children with a ratio of 2 teachers Acorns RM: 7 children with a ratio of 2 teachers Daisy RM: 18 children with a ratio of 3 teachers The incident which occurred, Child #1 sustained stitches from having door closed on finger. Child #1 was not present at the time of the visit. LPA Morales inspected where the incident occurred and made observations. LPA observed the door where the incident occur, close rapidly. LPA Morales took photos of where the incident occurred, as well as a video. LPA observed a newly installed Happy Hands Anti Slam Child Door Safety Finger Trap Stoppers. Page 1 of 2 During staff interviews LPA was informed that Staff #1 (S1), Staff #2 (S2) and Staff #3 (S3) had active supervision of the child who obtained an injury. When asked if Staff usually let children open the doors, S1 stated "No we have duties for them like door monitor. But a teacher always has the door open and they "assist." LPA Morales asked S1, S2, S3 and S4 if children are allowed to open doors, staff stated "No." When asked if S1 told Child#1 to not open the door, S1 stated  "at that moment I told them to come next to me, and at that moment they decided to open the door." Staff #2 informed LPA that they checked on Child #1, and noticed Child #1, was bleeding and was taken to the office. Per S2, they placed gauze on Child #1 finger. Staff #2 informed LPA that Staff #2, helped apply pressure on Child #1 finger, and was given an ice pack. Per Staff #2, parents were notified of the incident.  When asked what protocols have been put in place to ensure this does not occur again, S4 stated there will be new hydraulics installed for each door, so this incident does not occur again. LPA Morales spoke with Parent #1 regarding the incident, Parent #1 has no concerns. During the inspection, LPA obtained a copy of the children's roster, obtained a copy of internal "Ouch Reports," conducted interviews with Staff, interviewed Adult #1, took photos and videos and made observations of where the incident occurred, collected emails between facility and Adult #2. At this time, no further investigation is needed. There were no deficiencies or citations in today visit. Notice of Site was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Caridad Greco. Page 2 of 2
