Incident investigation·March 6, 2026

On 3/6/26 at 9:33AM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced, in-person Case management incident inspection.  The incident was reported to Community Care Licensing on 3/2/26 via phone.  The facility did not report the Unusual Incident to the Department within the required 24 hours of occurrence. The incident occurred on 2/25/26.  Per staff there were no visible injures and did not think it was necessary to call the department. Incident: A child was riding the bike with teacher and the child's foot slipped off the staff foot and went in between the wheel and the handle bar and sustained a fracture. Upon arrival, LPA was guided on a tour of the facility by Director Caridad Greco to whom the purpose of the inspection was announced. Census:  There were 82 children being supervised by 15 staff. Busy Bee RM: 15 children with a ratio of 3 teachers Sprouts RM:  13 children with a ratio of 2 teachers Sunflower RM: 16 children with a ratio of 3 teachers Rainbow RM : 12  children with a ratio of 2 teachers Acorns RM: 9 children with a ratio of 2 teachers Daisy RM: 17 children with a ratio of 3 teachers During the inspection, LPA obtained a copy of the facility roster, and conducted interviews with 2 staff, took photos and made observations of the bike and the area where the incident occurred. This incident needs further information and will be concluded at a later date. Exit interview conducted and report was reviewed with Director Caridad Grecor.  A Notice of Site Visit was given and must remain posted for 30 days
