What the inspector wrote

On 04/27/2026 Licensing Program Analyst (LPA) DeCondia Ferguson and Licensing Program Manager (LPM) Raul Navarro conducted an unannounced case management incident inspection. The purpose of today’s inspection was to deliver findings regarding the Unusual Incident Report (UIR) submitted to the Department on 03/18/2026 for the incident that occurred on 03/17/2026. The facility reported the Unusual Incident within the required 24-hour timeframe. LPA and LPM arrived at the facility at 08:30 a.m. and met with Director Caridad Greco. LPA and LPM toured the arrival area of the facility where children are held until they are escorted to their designated classroom, and a census was taken. During today’s visit, there were 23 preschool children in care, and 15 staff present along with the Director. The facility is licensed for 113 preschool children and consists of 6 classrooms. Hours of operation are Monday through Friday from 08:30 a.m. to 02:00 p.m. Description of Incident: On 03/17/2026, at approximately 11:30 a.m., during circle time, Child 1 (C1) approached Staff 2 (S2) and stated, “I put a bead in my nose.” When asked where the bead was obtained, C1 stated it was found on the carpet. The facility notified the child’s father, who initially indicated he would remove the bead; however, he later informed the facility he would seek removal on 03/19/2026. LPA interviewed all staff present at the time of the incident. Interviews were consistent and did not indicate that any staff observed C1 place the bead in his nose during circle time. Information obtained supports that staff were actively supervising children at the time of the incident and that C1 independently reported the incident to staff. LPA also interviewed C1. C1 stated he found a bead on the carpet, played with it, and placed it in his nose. C1 reported the bead was later removed by a doctor. LPA interviewed Child 1’s parent (P1) regarding the incident. P1 reported he was notified immediately by the facility via telephone and responded by going to the school. P1 stated he examined C1 and subsequently sought medical care at UCLA/Mattel Children’s Hospital on the day of the incident. P1 reported a follow-up outpatient appointment was scheduled, and the bead was removed on 03/19/2026 in a clinical setting by himself and a pediatric otolaryngologist. P1 stated C1 did not exhibit symptoms such as pain, breathing difficulties, discharge, or discomfort between 03/17/2026 and 03/19/2026. P1 reported the delay in removal was due to medical scheduling and indicated the situation was urgent but not emergent. P1 further stated he was adequately informed by facility staff and that communication regarding the incident was appropriate. P1 reported C1 remained out of care following the incident and returned to the facility on 03/23/2026. LPA conducted interviews, reviewed documentation, and gathered information related to the Unusual Incident. Information obtained indicated that at the time of the incident, multiple staff were present, children were seated during circle time, and appropriate supervision was being provided during a classroom transition. Staff responded immediately upon being notified by C1 and escorted the child to the office for assistance. Information obtained further indicated that beads used in a prior classroom activity were stored in the designated art supply room and were not maintained as accessible classroom materials at the time of the incident. Based on interviews, record review, and information obtained during the investigation, LPA Ferguson determined that staff took appropriate actions and there was no lack of supervision or other regulatory noncompliance. The Unusual Incident is closed. An exit interview was conducted, and the report was reviewed with Director Caridad Greco. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies were cited during today’s visit. This report shall be made available to the public upon request. LIC 9213 Notice of Site was provided and is required to be posted for 30 days. Appeal Rights provided.
