Page 2 On 9/6/2023 interviews of staff and director were initiated. On 11/21/2023 interviews of all relevant parties was completed. Based on record review, interviews of all relevant parties, no information was disclosed that the facility is not kept in a comfortable temperature, therefore the allegation is unsubstantiated. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove or disapprove of the allegations, therefore the allegations are found to be unsubstantiated. Exit interview was conducted and a copy of the report was provided.
