What the inspector wrote

On 04/28/2025, Licensing Program Analyst (LPA) Tyra Chavies\302\240 conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 04/07/2025. Upon arrival, LPA met with Director, Briana Minton. LPA informed director about the purpose of the visit and toured the facility. LPA observed 25 children being supervised by 5 teachers. Incident Details: C#1 arrived at school on 04/02/2025 with a bruise on check. At drop-off Guardian rushed child in the door and left before was able to observe bruise and conduct health check. CPS was notified on 04/03/2025. On 04/28/2025 LPA Chavies observed children in care, conducted interview with director, and received personnel documents. Based on interview conducted and review of personnel documentation, once teacher seen bruise on check, teacher immediately called CPS. Staff did adhere to the Health and Safety codes. The facility is being given a Technical Violation for failure to report the alleged incident within the regulatory timeframe. Exit interview was conducted and a copy of the report was provided to director, Briana Minton. Notice of site visit was given and must remain posted for 30 days.
