What the inspector wrote

Licensing Program Analyst (LPA), Dayna Chambers, conducted a Case Management Incident inspection to follow up on self-reported incident that occurred at 12:30PM on November 4, 2022. Facility hours of operation are: 7:00am to 6:00pm Monday through Friday. The Monterey Park South West Child Care Office received the incident report on 11/04/2022 by Director, April Behrendt. Report stated that on November 4, 2022 there was a medical incident. LPA met with licensee and confirmed the facility followed protocol and regulations during the incident. On December 15, 2022, LPA and Director toured the facility, at the time of the inspection all ratios were compliant according to Title 22 Regulations. There was a total of 60 preschools present during inspection. RM#1 \342\200\223 Frogs \342\200\223 Staff #1 and Staff #2 with 14 preschoolers, RM#2- Butterflies \342\200\223 Staff #3 and Staff #4 with 14 preschoolers, RM#3 \342\200\223 Ladybugs \342\200\223 Staff #5 and Staff #6 with 17 preschoolers, and RM#4 \342\200\223 Owls \342\200\223 Staff #7 and #8 with 14 preschoolers. Based on today\342\200\231s inspection, and interviews conducted, the facility followed the appropriate Reporting Requirements, notified Parents, and contacted 911. LPA obtained documents confirming parents were notified. Parent was called manually by the Director, April Behrendt. LPA Received copy of ProCare app printout of incident that notified the parent immediately. At this time based on the available information it does not appear this incident was the result of a Title 22 violation and the facility followed the appropriate regulations to care for the children in care. Upon receipt, Notice of Site Visit shall be posted. Notice of Site Visit shall be posted for thirty (30) days. Failure to maintain posting as required will result in a $100 civil penalty. An exit interview was conducted, and a copy of this report was provided to Director, April Behrendt. Notice of Site visit was issued.
