Deficiency is cited in accordance with Title 22 of the California Code of Regulations and/or Health & Safety Codes. An exit interview was conducted, a copy of this report, appeals rights and a notice of site visit were discussed and provided to Director.

    101220.1(g)(1)The licensee shall document each child's immunizations and shall maintain such documentation.....(1)This requirement includes updating each child's immunization record when the child is due to receive required immunizations after enrollment in the child care center. This requirement is not met as evidenced by: During record review, 1 child was missing 1 vaccine. LPA reviewed 4 children files who no longer attends the facility and found 4 out 4 children’s immunizations were missing several vaccines. This poses a potential risks to the health and safety of children in care.
