What the inspector wrote

On 05/07/2025 , Licensing Program Analyst, Cristina Castellanos conducted an unannounced case management inspection at the above-mentioned facility. Upon arrival LPA was greeted by Director Felicia Flores, who guided LPA on a tour of the facility both indoors and outdoors. LPA observed forty-three (43) children and seven (7) staff members providing care and supervision. On 04/17/2025 the El Segundo Child Care Regional office received an Unusual Incident/Injury Report (LIC 624) via mail from the facility. The Unusual Incident Report submitted informed the Department that during Karate Enrichment at approximately 11:16am the child named on the report (C1) ran into a table hitting her face and chin on the left side. C1s bloody cut was cleaned and attended to by Director Flores and staff member S2. Parents were immediately notified of the incident and picked up C1 as soon as possible. The day of the incident, Director Flores confirmed that one (1) adult staff member S1 as well as an Enrichment Instructor (E1) were upstairs in the Social Hall for a Karate Enrichment session. There were no citations issued during today\342\200\231s inspection. The Unusual Incident Report investigation will continue. An exit interview was conducted. A copy of this report (LIC 809) and Notice of Site Visit was provided to facility Director Felicia Flores.
