What the inspector wrote

On 11/20/2025 Licensing Program Analyst (LPA) Tyra Chavies, conducted an unannounced Annual/ Random visit at Journey of Faith- Preschool Center. LPA toured the facility indoors and outdoors with director, Nancy Kim. Days and hours of operation are Monday through Friday 7:00 AM to 6:00 PM. LPA Chavies confirmed the phone number is: (310) 374-0583 and email address: nancy.kim@journeyoffaith.com. There were 98 children in care being supervised by 11 Staff members. LPA toured inside and outside the facility. The preschool classrooms are located in two different buildings, building E and building D. To access the main entrance (Building E) a key fob is needed. This facility has 10 classrooms. In Building E there are the: Blue Bumble Bee Room, Yellow Bumble Bee Room, Red Tiger Room, Green Bumble Bee Room, Traditional Kindergarten Owls, Red Starfish Room, Blue Starfish Room and Green Starfish Room. In Building D there are the: Traditional Kindergarten Turtles Room, Purple Starfish Room and the Pre- Kindergarten White Tigers Room. All classrooms are being used. LPA toured the classrooms and bathrooms. Indoor furniture and equipment are free of sharp, loose and/or pointed parts. During the tour LPA, advised director that several classroom/bathroom ceilings must be repaired. LPA also advised director that bathroom must be in good condition during childcare hours.\302\240 LPA observed age appropriate toys, books, creative art work and cubbies in primary care areas. Capacity and limitations as specified on the license are being maintained. This facility does have pets (amphibian) in certain classrooms. The facility has a working carbon monoxide detector and the smoke alarms can be accessed throughout both buildings. LPA inspected and verified that there is a 2A10BC fire extinguisher on the premises. Per director, there are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, and other hazardous items are made inaccessible to children in care. LPA advised director that all non- prescribed medication must be made inaccessible to children in care. All poisons are kept in a locked storage area. There is currently no Incidental Medical Services (IMS) been given at this time. LPA toured the outside space. There is a parking lot separating both buildings. Director advised LPA that children attending classes in building D will walk around the parking lot with adult supervision to access the playgrounds in building E. There are four playgrounds at this facility. Playground #1 is currently not in use. The director advised LPA Chavies that infants and toddlers are currently using Playground #3, Playground #4 is used for 2 year olds and Playground #2 is being used for 3 and 4 year olds. LPA observed age-appropriate toys, bikes and areas around climbing equipment for cushioning material to absorb falls. LPA advised director that the cushioning material in playground #3 and #4 will need to be replaced. There are sandboxes at the facility. Director advised LPA that the sandboxes are cleaned by facility crew and that a water/bleach spray is applied when cleaned. Sandboxes are usually cleaned every morning before use. Director advised LPA that the facility does provide Lunch and AM/PM Snacks. Parents may bring their own meals as well as AM/PM snack. LPA did observe kitchen area. Pesticides are not being stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. LPA advised director that other similar toxic substances should not be stored where there is food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. LPA also advised that all food taken out of the original packing shall have expiration dates placed on new packing. Drinking water is available both indoors and outdoors upon request. Per Title 22, A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test. LPA received Lead Testing Results from June 2023 and test results confirmed there is no fixture showing an exceedance over 5.5 ppb. The facility is in compliance. POSTING REQUIREMENTS: License and other relevant notices are visible for public view and correctly posted on the wall (Facility Sketch, PUB 394, PUB 269, Menus, LIC613A, LIC 610A,). Fire and disaster drills are being conducted as scheduled every month. CHILDREN'S RECORDS: LPA Chavies reviewed 10 children's files. All files currently have all required Forms (LIC 700, 701, 995E, 627, LIC 9227)\302\240 and immunization records. LPA advised director that LIC 995 and LIC 9227 should be clipped out and given to the parent. LPA also advised director that for each nonprescription medication, the licensee shall obtain, in writing, approval and instructions from the child's authorized representative for the administration of the medication to the child. STAFF'S RECORDS: LPA Chavies reviewed 5 staff files. All files currently have all required forms ( LIC 501, 503, 508, 9052) and Finger print clearance. One or more persons have received a First Aid/CPR certificate. LPA advised director that several staff must retake the Mandated Reporter Training in order to stay in compliance. LPA also advised the director regarding Education Verification for certain employees . Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Commencing September 1 st , 2016, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. Law enacted by SB 277, beginning January 1 st , 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into childcare or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint. A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN). Per Title 22 of the California Codes of Regulations, the facility is being cited for the following: Technical Violations. ( Please see LIC 9102) An exit interview was conducted with Director, Nancy Kim. A copy of this report was read and given to Director as well as LIC 9213 (Notice of Site Visit form.) LIC 9213 is required to be posted for 30 days.
