What the inspector wrote

Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Case Management visit at the facility to follow up on incident reported to the department on 2/26/26, that occurred on the same date in regards to injury sustained by Child 1 (C1). Upon arrival LPA met with Facility Representative Tameka Bell who allowed entry in the facility, LPA inofrmed of the purpose of inspection. This facility has 2 classrooms Room 1 EHS (Early Headstart) and Room 2 Headstart. LPA observed 17 children napping and 4 staff supervising. The facility reported that on 2/26/26 at approximately 9:30am during outdoor playtime, Child 1 (C1) was playing and running around the grassy area by the concrete bike path. C1 ran and fell on the bike path causing a cut to C1's forehead. During this visit LPA interviewed Staff, reviewed records and inspected the play area where C1 sustained the injury. LPA observed there are no tripping hazards, the grass and bike path are in good condition and repair. Based on record review, interviews and observations, there there were 14 children presents in the playground supervised by 2 staff (S1, S2). S1 stated they were present and facing children while they played. S1 saw C1 playing with 2 other children, C1 fell and sustained injury, S1 immediately provided 1st aid and called child's authorized representative. Per interview, C1 received medical care and is recovering at home. Per S1, child will return to the facility with physicians note and staff will follow any restrictions as needed. C1 was not present during inspection Per Interviews and record review, it was determined that the facility is not in violation of Title 22 regulation. No deficiencies are cited during this visit. Notice of site visit was provided and Site Supervisor was reminded it must remain posted for 30 days. Exit interview conducted and a copy of the report was provided to Vicente Alvarez, Early Learning Center Manager.
