What the inspector wrote

On 09/24/2025, Licensing Program Analyst (LPA) Lisa Clayton arrived at the Calvary Church of Pacific Palisades unannounced, to conduct the Annual Random inspection. Upon arrival LPA Clayton was greeted by Director Brittany Mann. LPA Clayton observed 22 children in care being supervised by 9 fingerprint cleared staff. The facility operating hours are 8:30am \342\200\223 12:30pm Monday \342\200\223 Friday. The program provides water. LPA Clayton toured the facility indoors and outdoors for a Health and Safety Inspection. The Co-Op provides care to children ages 2 \342\200\223 4 years old, in rooms 145 and 148 only and uses the gated Preschool Play yard for outdoor play. LPA Clayton inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment in the classrooms are in good condition, free of sharp, loose, or pointed parts. The sinks and toilets are in safe and sanitary operating condition, and floors in the facility are clean and safe. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There are no pools, ponds, or any other bodies of water on the property. There are no firearms or ammunition allowed or stored on the premises. All disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible to children in care. LPA Clayton conducted a Health and Safety inspection of the food storage area and observed it to be free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is to be discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. LPA Clayton observed working Carbon Monoxide detectors and working smoke detectors in classrooms 145 and 148. There is a fully functioning pull fire alarm in classroom 145, and the pull alarm in classroom 148 is being updated and per Director Brittany, the installation is scheduled to be completed within the next 7 days. LPA observed fully charged 2A10BC Fire Extinguisher outside of classroom 145 and a fully charged 2A10BC Fire Extinguisher outside on the play yard. A Fire permit was issued by the City of Los Angeles Fire Department on September 1, 2010. LPA Clayton observed the facility disaster log indicating the drills are conducted according to Title 22 regulations. LPA Clayton reviewed 5 children\342\200\231s files and observed the files containing current contact information of parents and authorized representatives, and authorization for medical treatment, signed Parent\342\200\231s Rights and signed Personal Rights receipts. LPA Clayton instructed Director Brittany that Blue immunizations cards are to be completed by Center staff upon receipt of the child\342\200\231s immunization records. t LPA Clayton reviewed 5 employee files which were found to be complete and in compliance. LPA Clayton reminded Director Brittany that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA Clayton informed Director of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. LPA Clayton reminded Director to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, no Deficiencies were cited, and no Technical Violations were issued. An exit interview was conducted, the report was reviewed and provided to Director Brittany. LPA Clayton posted a Notice of Site Visit which must remain posted for 30 days.
