What the inspector wrote

On 1/13/26, Licensing Program Analyst (LPA), V. Wheatley conducted a case management inspection and met with licensee/director Eromie Kumarage. LPA observed 21 children napping on the premises. The children were supervised properly within required ratios. During an inspection and investigation, LPA learned from interviews that there were cases of head lice which were not reported to the Department. Based on Title 22 Regulations, two or more cases of head lice or any communicable disease must be reported to the Department. This is a Type B violation. See LIC 809D. Exit interview was conducted. A copy of this report was read and provided to the director. The Notice of Site Visit will be posted for 30 days.

    101212 - Reporting Requirements-Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. This was not met based on evidence, the director did not inform the Department of two or more cases of Head Lice in September and October 2025. Based on Title 22 Regulations, two or more cases must be called within 24 hours and a written report (LIC 624) must be submitted within 7 days.
