What the inspector wrote

Licensing Program Analysts (LPAs) Jeanette Estrada, Jonnisha Culbert and Portia Bowden conducted an announced inspection at the facility noted above and met with YMCA Program Manager, Jessica Quintero. The purpose of the inspection was to inspect and measure two bungalow classrooms added to the facility. Per YMCA Facility representative, the originally licensed room (Cafeteria) is undergoing remodeling projected to take about 3 to 6 months. Program Manager confirmed the remodeling was not reported to the Department within the required time frame. There were no children present during today's inspection. The facility is currently licensed for 56 children, ages 4.5 thru 12 years old. The before and after-school program is located at Gompers Elementary School. Per Program Manager, the program operates Monday through Friday from 6:30 am to 9:00 am, 1:00 pm to 6:00 pm for early dismissal days and 2:00 pm to 6:00 pm every other day. All areas on the facility sketch used by daycare children were inspected. The program will provide before and after school care in two bungalows, B-32 and B-33 which can be accessed through a gate on Hardwick Street. The program will utilize the gated outdoor play yard on the campus. Shade was observed in the outdoor play area. Per Program Manager, there will be a walkie talkie placed on the gate for parents to call to have the gate opened. Children will be signed in and out inside classroom B-33. Per Program Manager, transportation is provided by the YMCA and the vehicles used have the proper seat restraints for children. LPAs informed Program Manager that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Program Manager Jessica Quintero stated that they will be located in the locked storage cabinet in Room B-33. A fully equipped first aid kit is located in each classroom. There is an operational carbon monoxide detector in each classroom. Smoke detectors were not tested during this visit due to the smoke detectors being wired to the emergency pull down fire alarm. The fire extinguisher is fully charged and was last serviced in July 2024. The following documents were observed posted in a prominent, publicly accessible area in Room B-33: Page 2 Facility License, Menu, Child Passenger Restraint System Poster (PUB 269), Notification of Parents' Rights (PUB 393), Emergency Disaster Plan (LIC 610), Personal Rights (LIC 613A), Daily Activity Schedule, Earthquake Preparedness (LIC 9148), and Verification of Disaster and Fire Drills. Furniture and equipment were inspected for age appropriateness and good repair. LPAs observed a total of 79 hooks for children to place their belongings (backpacks). In classroom B-32 there were 36 hooks outside of the classroom and 14 inside. In classroom B-33 there were 29 hooks outside the classroom. In Room B-32 LPAs observed various pieces of furniture which per Program Manager will be removed before the start of operation for the YMCA program. In both classrooms, LPAs observed standard desks and chairs for children, various storage units and shelf units which will be accessible to children to obtain toys and materials. Telephone service, heating, lighting and ventilation were evaluated. The program utilizes a cell phone which is present at the facility with the staff. Both classrooms were observed to have central air/heating. Children will use the restroom in the 2000 building of the campus. Sinks and toilets were inspected for availability and good repair. General sanitation was observed. Per Program Manager, children will be escorted and supervised when using the restroom. Disinfectants, cleaning solutions, and other items that are hazardous to children, were made inaccessible to children in all areas . Director confirmed that there are no poisons stored at the facility. The facility provides the children with snacks daily. Per Manager, no meals are prepared or served at the facility. All kitchen, food preparation and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. At the time of inspection, LPAs did not observe storage containers for solid waste to have tight-fitting covers. Per Program Manager, the isolation area for ill children will be located in a section of either one of the classrooms and well children will be moved to another classroom. Per Program Manager, if an ill child requires the restroom, A staff restroom will be used. There was no napping equipment to be inspected. Availability of indoor drinking water was observed in both classrooms via water jugs and disposable cups. Water jugs are filled from plastic water containers which are maintained in a storage unit in Room B-33. The facility tested for lead in drinking water in water sources near the licensed cafeteria on 8/8/23 and no lead exceedances were detected. Page 3 LPAs measured the indoor activity space: Room B-32 and B-33. Total indoor activity space measured 1733.23 square feet, which is not sufficient to accommodate the licensed capacity of 56 children. Total capacity which can be accommodated with the measured space is 49 children. The program was advised to submit an updated LIC 200A-Application for a Child Care Center License, to reflect the capacity which can be accommodated. LPAs advised Program Manager the following items must be provided prior to approval of operation in the newly added classrooms: Items in Room B-32 must be removed (photo) Updated LIC 200A- Application for a Child Care Center License, to reflect the capacity which can be accommodated. Proof of waste bins in classrooms with tight-fitting lids Proof that Director Fernando Contreras has the required pediatric first aid/CPR AED certification. A technical violation for regulation 101212(c) is being issued today due to the remodeling of the cafeteria not being reported to the Department. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with Program Manager, Jessica Quintero.
