What the inspector wrote

On 09/25/2025 Licensing Program Analyst (LPA) Jonnisha Culbert conducted a case management at the facility noted above. LPA met with licensee, Fernando Contreras and stated the purpose of today’s visit. The purpose of the visit is to verify the current location where care is being provided. In 2024,it was reported to The Department that Gompers Elementary is undergoing construction, and as a result, required the temporary relocation of children. After The Department conducted an inspection of the relocation sites, they granted a license to provide care in rooms B-32 and B-33. With construction now complete, the Department is conducting a follow-up assessment to confirm where care is provided. Per licensee, rooms 28103 (formerly known as B-33) and room 28102 (formerly known as B-32) are the only rooms used to provide care to children. Prior to the change, children utilized the cafeteria. LPA observed children in both rooms (28103 and 28102). Based on the interview and observation made during the visit, it was confirmed that care for children is provided in rooms 28102 and 28103. No deficiencies were cited during today’s visit. Notice of site visit was given and must remain posted for 30 days. Exit interview conducted and the report was reviewed with licensee, Fernando Contreras .
