On 10/31/2025,\302\240 Licensing Program Analysts, LPA, Tyra Chavies conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 10/17/2025. LPA Chavies toured the facility and met with Director, Shireen Panjwani. Incident: On 10/16/2025 there was power outage 12:10 am, upon inquiring with Edison we found out there was an explosion and they were working on it but the power won't be back until the next day. On 10/31/2025 LPA observed children in care, interviewed director, received facility report, personnel roster and facility documentation. Based on interviews conducted and review of personnel and facility documentation, it appears the facility is adhering to the Health and Safety Code governing California. An exit interview was conducted, report was reviewed and provided to director. Notice of site visit was provided and must be posted for 30 days.
