What the inspector wrote

Licensing Program Analyst (LPA) Ashley Calderon conducted a Case Management Visit to review Plan of Corrections (POCs) and Update Facility Information due to changes in the home.LPA met with Facility Representative Nida Nofies who allowed LPA entry to the facility. Shortly after Licensee Ernestine Blanchard met with LPA Calderon and LPA disclosed purpose of the visit to both representatives. During visit LPA reviewed and observed POC's: Children files for Affidavit regarding Liability Insurance LIC282 and Parent Rights forms LIC955A, forms were placed in children files. LPA observed poisons stored in the locked garage, that has a key lock knob that was placed in the storage room located in the side yard area. Baby Mobiles were observed removed above infant crib. Staff had immunization records in file, POC letters generated. Baby jumper was not in use during visit and was placed in the back off limit area. LPA informed Licensee infant equipment like gliders, soothers, rockers, swings and baby walkers and/or similar equipment are not safe and not to use for sleep due to risk of suffocation for infants.LPA consulted with Licensee regarding Personnel who provides transportation for school age children, during today's visit Licensee was given the Transfer Request - LIC9182 to associate individual under this facility License, LPA reviewed individuals fingerprints reviewed to be cleared. Per Licensee, will mail it in to the Monterey Park South West Regional Office via mailing address provided.In addition, LPA Calderon consulted in Facility Roster list, per Licensee would like to remove late husband. LPA will make changes when in the office for the roster; Licensee provided letter to remove individual from the roster. LPA consulted in updating Application LIC279 and LIC279B was provided to License to send to the department mailing address provided. A notice of site visit was given and 30 days must be posted. An exit interview was conducted with Licensee Ernestine Blanchard
