Inspector’s narrative

What the inspector wrote

On 12/09/2024 Licensing Program Analyst (LPAs) Tyra Chavies and Judy Laureano, conducted an unannounced year Annual/ Required at Ocean View Preschool. LPAs met with Director Rosana Lieu and toured the facility indoors and outdoors. Days and hours of operation are Monday, Wednesday and Friday 7:00 AM to 4:30 PM and Tuesday and Thursday 7:00 AM- 12:30 PM. LPA Chavies confirmed the phone number is: (310) 514-1048. There were 22 children in care being supervised by 3 Staff members. AM/PM snacks are provided by the center. Breakfast and lunch are provided by parents. If a child leaves their lunch at home, the facility will provide the child a lunch. There is currently no Incidental Medical Services (IMS) being given at this time. There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. Classroom 1 and 2 are currently being used for child care. Classrooms 3 and 4 had no children in the space due to low in enrollment. This facility does a small chapel currently being used occasionally. Indoor furniture and equipment are in good condition, free of sharp, loose and/or pointed parts. The facility floors, bathroom and classrooms meet Title 22 Health and Safety regulations. LPA observed cots for sleeping, age appropriate toys and books in primary care area. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around climbing equipment have cushioning material to absorb falls.\302\240 All materials and surfaces accessible to children are toxic free. LPAs inspected and verified that there is a fire system connected throughout the facility. The facility does have a fire extinguisher which has an inspection date of on November 29,2023 on the premises. Facility has one or more functioning carbon monoxide detectors. The facility's children's individual disaster kits are currently expired with a expiration date of November 2024.\302\240 Director stated the new kits are set to arrive any day. Drinking water is available both indoors and outdoors upon request. Lead testing has not been done. Director has informed LPA Chavies, that the facility only uses bottled water for drinking water and to prepare meals. The following were also discussed with the licensing: Individuals who are 18 years of age must obtain a criminal record clearance.\302\240 Individuals within one month of their 18th birthday must be fingerprinted immediately . Failure to obtain a criminal record background check clearances prior to initial presence in the facility will result in an immediate $500.00 dollar civil penalty and $100.00 more per day until licensee is in compliance . Commencing September 1 st , 2016, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. New Immunization Requirement: Law enacted by SB 277, beginning January 1 st , 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into childcare or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint. The director, was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot line at 1-800-540-4000. Director was informed to call the CCL office and follow up with a written Unusual Incident/Injury Report (LIC 624B). Director was informed that any employee is to call CCL office and file an Unusual Incident Report when a child is injured at the facility. Also, Director was informed to send in written form (LIC 624B) to CCL Office within 7 business days. New Appeal Process : A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN). Per Title 22, of the California Code of Regulations, the following Technical Violation and Type B were discussed. (See LIC 9102 and LIC 809 D) An exit interview was conducted with Director, Rosana Lieu. A copy of this report was read and given to Director as well as LIC 9213 (Notice of Site Visit form.) LIC 9213 is required to be posted for 30 days.
