What the inspector wrote

On 2/20/2025 Licensing Program Analyst (LPA) Tyra Chavies, conducted an unannounced 3 Year Required visit at L.A Harbor College Child Development Center. LPA met with Director Gretchen Hayes and toured the facility indoors and outdoors. Days and hours of operation are Monday through Thursday 7:45 AM to 5:00 PM and Friday 7:45 Am to 3:00 PM. LPA Chavies confirmed the phone number is: (310) 233-4205. There were 42 children in care being supervised by 16\302\240 Staff members. PM snacks are provided by the center as well as breakfast and lunch. There is currently Incidental Medical Services (IMS) been given at this time. This facility currently has four classrooms ,Sea Stars, Dolphins, Guppies and Turtle and all are currently being used for child care. Indoor furniture and equipment are in good condition, free of sharp, loose and/or pointed parts. The facility floors, bathroom and classrooms meet Title 22 Health and Safety regulations. LPA did observe the facility to have pets that are in good condition (turtles and fish). LPA observed cots for sleeping and bedding will be washed at the facility. LPA also observed age appropriate toys and books in primary care area. The surface of the outdoor activity space is maintained in good condition and is free of hazards. LPA did advised director that teachers are to maintain one hundred percent supervision visually and audible at all times. There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. LPA inspected and verified that there is a fire system connected throughout the facility. The facility does have a fire extinguisher which has an inspection date of November 13,2023 on the premises. Facility has one or more functioning carbon monoxide detectors. First Aid kit was observed in each classroom. Drinking water is available both indoors and outdoors and upon request. Lead testing has been done even though building was built after 2010. There are two Exterior faucets CR 111 and CR 120 that show a reading over 5.5 in November 2024. This facility does have bottled drinking water that is given to the children upon request. POSTING REQUIREMENTS: License and other relevant notices are visible for public view and correctly posted on the wall (Facility Sketch, PUB 394, LIC 9148, LIC 610A) except for the Fire Department Safety Permit. Fire and disaster drills are being conducted as scheduled every six months. CHILDREN'S RECORDS: LPA Chavies reviewed 10 children's files. Most of the documents were completed. LPA Chavies advised director to conducted a file review to ensure all documents are completely filled out. STAFF'S RECORDS: LPA Chavies reviewed 8 staff files. All files are up to date. One or more files are missing mandated reporter training. Director is missing the 8 hours of Health and Safety training, child nutrition and lead poisoning training as well as the Component ft- Operations and Record Keeping. LPA Chavies has advised director that the facility will be given a week to provide documents. The following were also discussed with the director: Individuals who are 18 years of age must obtain a criminal record clearance.\302\240 Individuals within one month of their 18th birthday must be fingerprinted immediately . Failure to obtain a criminal record background check clearances prior to initial presence in the facility will result in an immediate $500.00 dollar civil penalty and $100.00 more per day until licensee is in compliance Commencing September 1 st , 2016, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. New Immunization Requirement: Law enacted by SB 277, beginning January 1 st , 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into childcare or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint. New Appeal Process : A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN). Per Title 22, of the California Code of Regulations, the following Technical Assistance and\302\240 Violations were discussed. (See LIC 9102) An exit interview was conducted with Director, Gretchen Hayes. A copy of this report was read and given via email due to printer issues to Director as well as LIC 9213 (Notice of Site Visit form.) LIC 9213 is required to be posted for 30 days. A copy of this report will be mailed as well to the facility.
