What the inspector wrote

On 1/30/26, LPA observed (3) toddler classrooms combined for lunch in Room 137, ratio was 2:8 with 2 interns from College Corp. For the preschool side ratio, Room 110 ( 2:15 with 1 intern), Rm 114 (2:12 and 1 intern) and Rm 116 (1:4 with 1 intern); facility was found in ratio. Both visits, LPA did not observe ratio conflict that can lead to lack of supervision. Based on interview from 1/21/26 with Director C.Manansala who informed LPA addition to facility staff, facility has 20 intern personnel's assisting in both the toddler and preschool side. Director and teachers #1-4, informed LPA Calderon teachers due zoning, position themselves in different areas to observe/supervise children and informed LPA are always within ratio and had no ratio concerns. In addition, LPA Calderon interviewed 4 out of 5 parents. The following is based on parent interviews informing LPA Calderon facility is good with supervising the children and with ratio and facility has interns to assist. Regarding the allegation: 'Staff are not properly reporting incidents involving the daycare children' is unsubstantiated. The investigation consisted of LPA Calderon on 1/21/26 obtained supportive documents on: written oopsie reports for preschool side (there was none for toddler side), Facesheets of children indicating if they do/don't have oppsie reports (in the toddler side), procedure instructions for reporting Unusual Incident Reports (UIR), and Handbook pages on Care and Supervision, Injuries and Licensing information and Ratio. Based on review of supporting documents 3 Oppsie Reports for the preschool side, parents were notified of incident and signed off on incident report provided, children were provided necessary treatment if needed at the facility( example: ice packs) and no incidents per review were necessary to report to licensing as they did not required professional medical attention. Based on review of 5 toddler Facesheet Checklist reviewed on 1/6/26 by the facility, no child had oopsie reports recorded. Per review, of Facility policy on reporting UIR's events listed on LIC624 require facility to immediately call parents via phone and Care Connect and per handbook if minor injuries (scrape, bruise, bump) teacher reports incident on Care Connect and/or Class Dojo and major injury involve parents get a written oppsie report, document on Care Connect, Class Dojo and facility reports to licensing. Per Interview, with Director Manansala informed LPA Calderon incidents/injuries are reported to parents in 3 Communication Networks: Care Connect, Class Dojo and written Oppsie reports. 4 out of 4 teachers informed LPA they use Class Dojo App to inform parents about incidents/injuries. Director and 4 teachers interviewed informed LPA, staff would be notify parents depending on incidents via phone, Class Dojo, Care Connect and/or in person. In addition, LPA Calderon interviewed 4 out of 5 parents. The following is based on parent interviews: facility communicates with parents by using Class Dojo as a communication platform, facility keeps parents posted of incident/ injury that occurs, depending on the incident parents informed LPA facility may report over the phone, or on Class Dojo app or in person, and parents during interview informed LPA they facility has good communication. Moreover, due to lack of information from the complaint, the investigation revealed that based on interviews with staff and parents, observations made during the facility tours, and copies of supporting documents obtained, that there was no evidence to indicate allegations: staff do not provide adequate supervision of the daycare children and to indicate staff are not properly reporting incidents involving the daycare children. Although the allegation may have happened or is valid there not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days consecutive days. Exit interview conducted and a copy of the report and appeal rights were provided to Director Candance Manansala.
