On Wednesday October 29, 2025, Licensing Program Analyst (LPA) Joanne Solorio-Campos conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Veronica Proctor, Assistant Director, who guided LPA on a tour of the facility. Census was taken. On October 9, 2025, it was self-reported to the department that a child sustained a head injury during transition to nap time. Per Director, the child was jumping with their blanket as they walked to their mat and got tangled in it causing the child to fall and bump their head on the floor. Ice was applied and the child was comforted. Child’s parent was called and given an ouch report and concussion protocol at pick-up. The child remained at school and also returned the next day. Based on observation and review, LPA confirmed that the facility responded appropriately to the incident. No deficiencies being cited on this date. An exit interview was conducted with Veronica Proctor, and a copy of this report was provided along with Appeal Rights. Notice of Site Visit was provided and must be posted for 30 days.
