On Wednesday October 29, 2025, Licensing Program Analyst (LPA) Joanne Solorio-Campos conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Veronica Proctor, Assistant Director, who guided LPA on a tour of the facility. Census was taken. On October 7, 2025, it was self reported to the department that 5 children were confirmed to have Hand Foot and Mouth Disease(HFMD). During the inspection, it was reported that staff immediately informed parents of an exposure to Hand, Foot, and Mouth Disease (HFMD) by issuing and posting an exposure notice to all families enrolled. The facility also implemented enhanced sanitizing measures, and maintenance was directed to ensure that all areas were properly cleaned and disinfected. Since the initial report, an additional 8 children were confirmed to have HFMD, bringing the total to 13 children total confirmed. Children returned to care with doctor’s clearance notes authorizing their return and by facility’s registered nurse also observing children’s return. No other cases were determined. Facility reported the exposure to the Department of Public Health as well per title 22 regulations. LPA Solorio-Campos reviewed related documentation and provided the facility with Department of Public Health resources to ensure preparedness for any future communicable disease exposures. Based on observations and review, LPA confirmed that the facility responded appropriately to the incident and is in compliance with Title 22 regulations regarding reporting requirements and Health and Safety. No deficiencies being cited on this date. An exit interview was conducted with Veronica Proctor, and a copy of this report was provided along with Appeal Rights. Notice of Site Visit was provided and must be posted for 30 days.
