Incident investigation·June 2, 2026

On Tuesday June 2, 2026, Licensing Program Analyst (LPA) Joanne Solorio-Campos conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Bianca Ceibrant, Program Director, who guided LPA on a tour of the facility. Census was taken. On April 29,2026, it was self-reported to the department that a child sustained an arm injury during outdoor play time. Per Director, the child was walking on the raised step pods in the playground when they lost their balance and fell. Ice was applied and the child was comforted. Child then stated his elbow hurt and had limited range of motion. Child’s parent was present in the facility in another classroom. The facility provided parent with an ouch report and parent took the child to the doctor for evaluation and later informed the facility that the child was given a cast. Per Director, the step pods were previously inspected for the presence of any water, however, no water was observed. The teachers present stated the child simply lost balance. Child returned to school on 5/4/26 with restrictions. The facility also created an individualized health plan for the child. Since the incident, the child’s cast has been removed; however, the child remains on limited activity restrictions. Based on interviews, and observation, it was determined that staff were unable to get to the child on time to prevent the injury from happening however, there was at least one staff member who visually saw the incident occur; therefore, there is no preponderance of evidence to determine that a regulatory violation occurred, the incident was determined to be an accident. No deficiencies or civil penalties are being cited at this time. An exit interview was conducted with Bianca Ceibrant, and a copy of this report was provided along with Appeal Rights. Notice of Site Visit was provided and must be posted for 30 days.
