On Tuesday June 2, 2026, Licensing Program Analyst (LPA) Joanne Solorio-Campos conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Bianca Ceibrant, Program Director, who guided LPA on a tour of the facility. Census was taken. On May 14,2026, it was self-reported to the department that 911 was called due to a child having difficulty breathing. Per Director, the child has an individual health plan in place and an inhaler at school. On 5/13/26 the child stated their chest was hurting and had difficulty breathing. The facility could not provide the inhaler as the child had received the medication earlier that morning at home. The facility called 911 and parents. Paramedics arrived and evaluated the child and declared the child did not need to be transported to the hospital. Childs’ father informed the facility that child’s grandparent would do the pick-up. On 5/14/26 child’s mother informed the facility that she was going to keep the child home for a few days. Child returned to school on 5/19/26. Director stated that the facility is following the doctor’s instructions regarding the medication provided and their health plan to which there is an 8hr medication time frame. The morning of 5/13/26 the mother of the child stated they provided the inhaler to the child at home. The facility informed the child’s mother that if the child had any difficulty breathing before the medication time frame, they could not provide the inhaler and would have to call 911. The facility Health Coordinator discussed with the mother the possibility of adjusting the medication time frame, however the mother stated she was comfortable with the current plan in place and expressed that making changes to the plan would take too long and also stated the child was soon leaving the program to transition to TK. Based on interviews, and observation, the facility acted in accordance with their protocol, therefore, there is no preponderance of evidence to determine that a regulatory violation occurred, the incident was determined to be an accident. No deficiencies or civil penalties are being cited at this time. An exit interview was conducted with Bianca Ceibrant, and a copy of this report was provided along with Appeal Rights.
