On 8/13/2025 at 9:15 am Licensing Program Analyst (LPA), Claudia Kam made an unannounced required visit at above licensed facility. LPA disclosed the purpose of the inspection and was granted entry into the facility. LPA met with Program Manager/Facility Reprentative (FR) Gabriela Kayacan and was taken on a guided tour of both indoors and outdoors of the facility. LPA observed Teacher-child ratios, and staff names recorded. All center staff that was present during today\342\200\231s inspection had fingerprint clearance but 3 staff were not associated to the designated license number. Care and supervision evaluated it is determined that the basic needs and personal rights of children are met and appropriate. The hours of operation are 9 am \342\200\223 3 pm, with 2 half day programs 9:00 am -12:00 pm and 12:30pm - 3:30 pm, Monday, Wednesday and Thursday with make up days on Tuesday and Friday as needed. Facility is currently licensed for a capacity of 41. At the initial start of the site visit, there is a total census of 25 children and a total census of 10 teachers/staff present. The facility is made up of 2 classrooms. Room 2: had 13 children and 5 staff, Room 3: had 12 children and 5 staff present at time of inspection. LPA observed all posting requirements for operation on the posting board and the following documents were observed LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent\342\200\231s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Page 1 of 4 Due to half day program no napping equipment is used for the children in care. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. LPA observed cubbies with wooden toys, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in each classroom. LPA observed backpacks for emergency evacuation kept in each classroom. First Aid supplies are kept in the staff lounge which is accessible to the classrooms and were inventoried and were found to be in compliance with regulation. Facility provides, snack and lunch, menus were reviewed at time of inspection. All food items delivered daily to the facility by IILA. Food and snack items are properly stored. Food staging area was toured and found in compliance for safety, cleanliness, and proper storage. Drinking water is provided via water dispenser. Carbon monoxide detectors and smoke detectors are present in the facility. LPA confirmed with Facility Representative that detectors at time of inspection are in operable condition. Fire extinguishers were observed as serviced on 1/13/2025. Last fire drill was conducted on 6/23/25. LPA observed outdoor play area which consists of a large shade structure with play structure underneath and rubber cushioning. The play area has a retractable awning for the table area. Tables are used for activities and were observed to be in good condition. LPA in addition observed a swing, play house, ride on toys and raised garden beds. Outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. Required shade and fencing were inspected. LPA advised Director that the children need to be always within the direct care and visual supervision by the teachers. Per Director, the children have access to drinking water via a water jug and disposable cups. Water jug was observed present at time of inspection. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection. LPA reviewed with Facility Representative the LIC 311A, Records to Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Staff and Children\342\200\231s Records were reviewed. Criminal Record Clearances were reviewed for adults. LPA observed current CPR and First Aid Certification for center staff. CPR card expires on 4/2027 for Staff Member. Mandated reporter has expired for all staff members. Page 2 of 4 Mandatory Forms for the children\342\200\231s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov . to ensure the most current information regarding regulations for all those who assists in providing care at the facility. The following were discussed with the Facility Representative: The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Page 3 of 4 Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given and must remain posted for 30 days Exit interview conducted and report was reviewed with facility representative Gabriela Kayacan.
