On September 20, 2023 at 8:00AM, Licensing Program Analysts (LPAs) Monique Ayala and Staicy Perry conducted an unannounced required 1-year inspection. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with Lead Teacher Anabell Ramirez who guided the LPAs on a tour of the facility. This is a preschool age program which operates Monday, Wednesday, Thursday and Friday from 8:05AM to 2:38PM and Tuesday from 8:05AM to 1:38PM. Per the Lead Teacher there are 6 children enrolled. The following classrooms were inspected: Classroom 1: 5 children with 2 teachers. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision. Licensed facility is within the conditions, limitations, and capacity specified on the license. The classroom furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. The facility provides all meals to children in care. The meals are provided by the elementary school cafeteria. The kitchen was observed to be clean and the cleaning compounds/solutions were found to be stored away from the food. LPA's asked the lead teacher, office staff and Plant Manager if the facility/classroom was tested for lead. Per Lead teacher and Plant Manager the classroom was tested for lead, but LPAs were unable to review report due to the lead report not being found. Children bring their own water from home and the lead teacher provides water bottles. LPA observed that there are functioning smoke and carbon detectors in the facility. LPA observed a fully charged and serviced fire extinguisher. Emergency disaster drill was last conducted on 09/08/23. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. LPA's observed trash and debris in the play yard. LPA's observed trash can located in the play yard with food, but the trash can had no lid. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPA reviewed staff records, for Lead Teacher; files were complete. LPAs were unable to review staff records for Aide due to the aide not having a file at the facility. Children\342\200\231s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. LPAs observed for Child #1 (C1), there is no Physicians Report (LIC701) on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. SB792 (Immunization Requirements for Staff and Employees) was discussed with the Lead Teacher. The Lead Teacher and staff do currently have immunization documentation. AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Lead Teacher was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Lead Teacher was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE. LPA referred lead teacher to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. LPAs referred lead teacher to review PIN 21-21.1 CCP Based on the LPA\342\200\231s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and a copy of the report was provided to Lead Teacher along with appeal rights.
