What Happened
Case management investigation of unusual incident from 6/17/2025 where child was left unattended on playground. Type A and Type B deficiencies cited. Parents must receive copies of licensing report.
Observations
- Child left unattended on play yard after being taken to restroom near Classroom 2B on 6/17/25 at approximately 10:15 AM
- Aide returned without child, who was later found on play yard by Coach Alma
- Duration of time child was left alone unknown
- Parents notified upon picking up child
- Type A and Type B deficiencies cited in accordance with Title 22 California Code of Regulations
